Meeting Attendance Request

Are you a student who needs to attend a meeting of a professional organization as part of your program requirements?  We'd love to have you!  We receive a high volume of these requests, so please fill out the form below so we can accommodate your request.  Please submit your request at least 1 week prior to the meeting you'd like to attend.  A representative will be in contact with you shortly to confirm your attendance at an upcoming PHNAC meeting.

Please refrain from calling our office to request permission to attend an upcoming meeting.  We receive too many inquiries to handle them all via telephone.
